Children's Case Management Services

Mental Health Case Management 

MH Case Management services help individuals with a documented mental health diagnosis (e.g. psychiatric evaluation, IEP for children, etc.) with obtaining and coordinating other resources, including medical, social, housing, educational, or other needed services for the child and his/her family.  Transportation of individuals is not a covered case management service.  Documentation needs to be no more than a year old.  

Agencies that provide MH Case Management services for children are:

• Penndel Mental Health Center supports all ages for individuals living in the southern Bucks County area.
• Merakey  Supports all ages for individuals living in the southern Bucks County area.
• Family Services of Bucks County supports individuals 18 years of age and older living in the southern Bucks County area.
• Lenape Valley Foundation supports all ages for individuals living throughout Bucks County with a focus in the southern and central area.
• ACCESS Services supports individuals aged 16 through 26 living throughout Bucks County.
• St. Luke's Penn Foundation supports all ages for individuals living in the northern Bucks County area.
• Child and Family Focus supports individual aged 16 through 26 living throughout Bucks County.
