Individuals found eligible for ID/A registration receive Supports Coordination services. Supports Coordinators build relationships with individuals and their families and help them locate, coordinate and monitor needed services and supports.
The purpose of Supports Coordination is assess and locate both natural (unpaid) resources and paid services, when needed.
