Individuals found eligible for ID/A registration receive Supports Coordination services. Supports Coordinators build relationships with individuals and their families and help them locate, coordinate and monitor needed services and supports.
The purpose of Supports Coordination is to assess and locate both natural (unpaid) resources and paid services, when needed.
Individuals who receive Supports Coordination services have the right to change which Supports Coordination agency serves them. If you would like to change which Supports Coordination agency serves you or your family member, please call our office at 215-444-2800.
Please contact Tanya Schoeffling with questions regarding Supports Coordination at 215-444-2840.
