Required Documentation

There are a number of documents that are needed for the ID/A intake process in order to determine eligibility and begin initial services.  These documents include:

  • Birth certificate
  • Social Security card
  • A physical examination form signed by a doctor
  • School evaluation records (such as IEPs and psychological evaluation reports) 
  • Guardianship records (if applicable).

You will be provided any forms that need to be filled out for intake.  Gathering some of these records beforehand may save you time during the intake process. 

Below is a full list of all of the documents that are needed for eligibility determination.  We recommend printing out or downloading a copy so you can track what records you already have and which ones you still need for intake.  

Please note there are different documentation requirements for ID services than there are for Autism services.  Make sure to select the list that you need.
